Beginning this year, Subordinate and Pomona Officers have a choice when submitting reports:

You may:

1. Print and complete your Subordinate/Pomona Officer report and then mail (U.S. mail) it to State Headquarters or
2. Fill out the form(s) online and submit them via electronic mail to State Headquarters.
